New patients call the office or send a request, and we confirm the appointment directly — first visits are longer, and neurofeedback and decompression appointment times vary.

What to expect

    Request or call

    Send a request through this site or call the office. We will ask a few questions to make sure your appointment type and length are right.

  2. 2

    Secure intake

    We send you your intake paperwork so you can complete it on your phone or computer before you arrive. Printable copies are available if you prefer paper.

  3. 3

    Consultation

    We go through your history: the complaint, what makes it better or worse, your work, sleep, activity and previous care.

  4. 4

    Examination

    Orthopaedic, neurological and movement testing appropriate to your complaint, with imaging or referral when the findings call for it.

  5. 5

    Report of findings

    A plain-language explanation of what we found, what we recommend, how long it should take and what it will cost.

  6. 6

    Treatment

    Care usually begins the same visit when it is appropriate, along with what to do between visits.

Patient forms

Complete your forms before you arrive

Once your appointment is confirmed, we send you a secure link to your intake paperwork. You can complete it on your phone or computer, save part-way through, and finish later — no printing, and nothing to fax.

Prefer paper? Printable copies are available on request, and Spanish-language forms are available either way. Just tell us which you would like when you book.

Please do not send medical details through the website request form or ordinary email. We collect health information through our secure intake process or in the office.

Payment & insurance

Before you begin care

Our front desk will check your benefits and explain your expected costs at the report of findings, before you commit to a plan. Coverage differs considerably between plans, particularly for decompression, neurofeedback and wellness services.

How do new patients book an appointment?

Call the office or send an appointment request through this site. A team member confirms the time with you directly, because first visits are longer and some services have specific appointment windows.

How long is the first visit?

Longer than a routine visit, because it includes a consultation, examination and a report of findings. Allow extra time, and arrive a few minutes early if you have paperwork to finish.

Will I be treated on the first visit?

Often yes, when the examination supports it and you are comfortable proceeding. If we need imaging or a referral first, we will explain why.

What should I bring?

Photo ID, your insurance card if you plan to use insurance, a list of medications, and any recent imaging or reports related to your complaint.

What should I wear?

Comfortable clothing you can move in. Loose trousers and a t-shirt work well for most examinations.

Do you see children?

Yes. Children are examined and treated with age-appropriate, low-force technique, and a parent or guardian stays for the whole appointment.

Are forms available in Spanish?

Yes — intake forms are available in Spanish. Tell us when you book and we will send the Spanish version, or ask at the front desk.
